Generating electronic (optimised) version

Currently, the output is optimized for printing. An output optimized for electronic consumption (PDF viewer) is needed. Ideally configurable by a class option. What is needed:

• odd/even pages with the same header/footer
• omit inserting blank verso page before a chapter begins
• [] page numbering consistent with physical PDF page number (to be discussed) - decided it will not be done.
